Overview
If you purchased, paid for (regardless of whether you paid or a third party paid on your behalf) and received a credit score or credit monitoring Offering from an
Equifax website (Equifax.com) or entity, or a Fair Isaac website (myFICO.com) or a reseller of the Suze Orman FICO® Kit
between November 19, 1999 and February 8, 2007, you are eligible to receive a benefit from the Hillis/Slack Settlement.
The Court approved the Settlement at the Fairness Hearing on June 4, 2007. The Settlement resolves two lawsuits in which claims were made that Equifax Consumer Services, Inc. (“Equifax”) and Fair Isaac Corporation (“Fair Isaac”) (collectively, the “Defendants”) violated the federal Credit Repair Organizations Act and similar state laws. Under the Settlement, Defendants deny that they are liable, but have agreed to make certain changes to the Offerings and to the advertising and marketing of the Offerings.

Class Member Options
Class Members as defined in the Settlement who received an Offering from the
Defendants are entitled to receive 3-6 months of free Score Watch.
Participate in the Settlement
IF YOU AGREE WITH THE SETTLEMENT, AND WISH TO RECEIVE THE FREE SCORE WATCH, YOU MUST
SUBMIT AN AUTHENTICATION FORM BY NOVEMBER 20, 2007. The easiest way to submit the Authentication Form is to do so online at
Authentication Form.
There is no cost associated with the submission and it will be accomplished electronically. You may mail in an
Authentication Form but that will cost you
postage. Authentication Forms must be submitted online or be postmarked no later than November 20, 2007.
Exclude Yourself From the Settlement
The deadline for Class Members to exclude themselves has passed.
Object to or Comment on the Settlement / Attend the Hearing
The deadline for Class Members to object or comment on the Settlement has passed.
Do Nothing
If you do nothing, you will automatically be included in the Settlement Class and give up your right to
file your own lawsuit, but to receive the free Score Watch you must submit an Authentication Form
by November 20, 2007. If you mail your Authentication Form, it must be postmarked on or before November 20, 2007. If you submit your claim form online, it must be electronically submitted by November 20, 2007.
Fairness Hearing
A hearing was held before the United States District Court of the Northern District of Georgia, on June 4, 2007. At the hearing, the Court approved the Settlement, approved the award of $4,000,000.00 in fees and expenses to Class Counsel, and approved the Incentive Awards in the amount of $7,500.00 to Class Representatives Hillis and Slack. An appeal was subsequently filed. The appeal has been resolved.
